Ready to get a doggy companion? Yay, you're on the track towards doing so! Please complete this puppy adoption application to assist us in placing our puppies in their forever homes. Please ensure that ALL fields are complete and a FEW pictures uploaded!

There are no right or wrong answers!!! The more information you can provide to me about you, your family, your home and lifestyle, the better we can decide and approve your application. Our Puppy Adoption Application will help me get to know a little about your family, lifestyle, and what you are looking for, as far as color, gender, and the time frame you want a puppy. Without it, I don't know if I can help you, so please take your time and fill it out with as much information as you can give me. The only way I know if one of my Cavaliers would be a good fit in your home, is with the information you give me. Cavaliers are special little dogs, and not suitable for all homes. Tell us a little bit about yourself and your family. Do you have any children? If yes, how many and what are their ages, and what history do they have with dogs? What activities do you enjoy, etc.? Do any family members have allergies to animals? If yes, to what and how severe?

Have you ever owned a Cavalier King Charles Spaniel? What made you interested in adopting a Cavalier King Charles Spaniel puppy? What will you do with your Cavalier King Charles Spaniel during vacations or extended traveling?

Do you have a fenced yard? If not, how do you plan to protect your Cavalier when he/she is outside? Are you aware that Cavaliers are inside dogs and cannot handle hot or cold weather? Do you have a pool? Is it fenced?

Do you have any current pets? If yes, what pets do you have (breed, sex, age)? And what are their spay/neuter status? Also, please list dogs previously owned including breed, how long did you have them, how long were they with you, and what happened to them (why you no longer have them)?

Do you have a current or previous Veterinarian we may contact for a referral? Please list contact information.

Are you committed to care for this dog’s needs for his/her lifetime? (Such as but not limited to: Food, shelter, love, patience, training, light daily exercise, shots, heartworm/parasite (flea, tick) and any and all medical needs). For medical needs please let us know if you need info or recommendations with Pet Insurance.

Your puppy will be considered your companion and comes with AKC limited registration which means there are NO breeding rights! AKC limited registration paperwork will be sent to you after you have sent us documentation from your veterinarian that the puppy has been spayed/neutered. On rare occasions we find an exceptional puppy we will offer our Guardian program. If you are interested in being a Guardian, we can discuss when we talk. Thank you for your understanding. *

Yes, I understand that the puppy comes with AKC limited registration and you agree to spay/neuter your puppy at 8-months old.

By purchasing a puppy from us you agree if there is any time or any reason why you're no longer able to take care of the puppy during its lifetime you will bring the puppy back to us the breeder? Are you willing to do this? Please don't take a dog pound, or give the puppy away. We will ensure a great placement as we have their best interest at heart!

I have answered the questions to the best of knowledge and truthfully. I understand that if I have not represented myself truthfully and under false pretenses I must return the puppy to the breeder without any recompense.
